Clinical Pharmacist in General Practice
We are seeking a clinical pharmacist to join our friendly team. This is an exciting opportunity to extend our clinical pharmacist role in practice for the benefit of our staff and patients.
You will work as part of a multidisciplinary team to manage repeat prescription authorisations and reauthorisations, acute prescription requests, management of medicines on transfer of care, and systems for safer prescribing and optimisation. You will be expected to perform medication reviews, support management of patients with long-term conditions, provide telephone consultations, liaise with secondary care providers, take part in prescribing meetings with practice staff and liaise with the ICB Medicines Optimisation Team to help us meet requirements of the local incentive scheme.
You will be working alongside supportive clinical colleagues and have the support of a fantastic administration team. We are a forward-thinking training practice and would welcome an enthusiastic and motivated pharmacist who can actively contribute to the continued development of the practice.
Applicants will have a post graduate diploma or higher degree in clinical pharmacy and must be willing to become an independent prescriber. We desire a self-motivated pharmacist who is confident in working independently and as part of a team, able to prioritise workload and willing to take on responsibility for decision making. Previous experience of working in a GP practice is essential.
Pay: From £50,500.00 per year
