Job Introduction
As a compassionate and dedicated AOS Senior Adult Outreach Practitioner, you will provide person-centred support to adults who have either received an autism diagnosis or are awaiting assessment, helping them develop a positive understanding of their neurodiversity and identify practical strategies to navigate challenges and achieve their personal goals.
Alongside managing your own caseload of adults, you will step into a senior role; supporting Practitioners with line management, caseload supervision, and day-to-day guidance, as well as supporting the leadership team to ensure the service meets commissioning expectations.
Using a solution-focused and strengths-based approach, you'll promote prevention and early intervention, reduce inequalities in access and outcomes, and embrace co-production; working in partnership with individuals, families, carers and communities to develop solutions that reflect lived experience.

Main Responsibilities

Delegated Management: Manage and supervise a team of outreach practitioners, providing guidance, support and caseload supervision, ensuring team members are trained, motivated and equipped to deliver high-quality care.

Training and Development: Provide training and mentoring to outreach practitioners, supporting their ongoing professional development.

Quality Assurance: Monitor and evaluate the effectiveness of interventions and care plans, participating in quality assurance processes and service improvement initiatives.

Personalised Support: Manage a caseload of individuals, delivering person-centred support that reflects their strengths, needs, aspirations and preferences, while working proactively to prevent unnecessary escalation of needs, including hospital admissions.

Family-Centred Planning: Work collaboratively with individuals and their families, where appropriate, to develop personalised, person-owned support plans that are flexible, responsive and regularly reviewed.

Collaborative Communication: Build and maintain positive working relationships with families, professionals and partner agencies to facilitate open, transparent and coordinated support.

Adapted and Creative Communication: Use autism-informed, creative and flexible communication approaches to ensure every individual's voice is heard, understood and reflected in their support.

Empowerment and Independence: Promote a strengths-based, solution-focused approach that builds confidence, resilience, self-advocacy and independence.

Participation in Multi-Agency Meetings: Attend and contribute to Safeguarding and Multi-Disciplinary Team (MDT) meetings, providing an autism-informed perspective.

Supporting System Navigation and Change: Identify and challenge barriers preventing individuals from accessing the support they need.

Prevention, Community Inclusion and System Integration: Maximise the use of community assets, social prescribing and partnerships across health, social care, education, housing and the voluntary sector to deliver coordinated, whole-system support.

Ideal Candidate

Driving licence and access to vehicle for work purposes.

Relevant undergraduate degree or equivalent professional qualification, or demonstrable experience working at degree level within a related health, social care, education or community setting.

Experience of working with autistic individuals and their families, with an understanding of the opportunities and challenges they may face in daily life.
-
Experience of providing supervision, line management and caseload supervision support to colleagues.
-
Experience of contributing to service development, quality improvement or performance monitoring activities.
-
Excellent verbal and written communication skills, able to build positive relationships with individuals, families and professionals from a range of backgrounds.

A genuine commitment to strengths-based, solution-focused practice.
-
Creative, flexible and autism-informed approach to communication and problem-solving.
-
Experience of working collaboratively within multidisciplinary and multi-agency environments.

Strong organisational, prioritisation and time-management skills to manage a caseload and maintain accurate records.

Knowledge of safeguarding legislation, policies and procedures.
-
Understanding of prevention, early intervention, co-production and community asset-based approaches.
-
Area based across Southend, Essex and Thurrock, with agility to travel to meet the needs of individuals.
