About the Role
Herefordshire Council is looking for a qualified Adult Social Worker to join their Young Adults (Transitions) Team.
This is a fantastic opportunity to make a real difference in the lives of young people aged 18–25 as they move from children’s services into adult social care. You will play a vital role in supporting safe, positive, and empowering transitions.
Key Responsibilities
- Conduct strengths-based assessments and identify eligible needs under the Care Act 2014
- Develop personalised care and support plans that promote independence and wellbeing
- Carry out Section 42 safeguarding enquiries
- Complete Mental Capacity Act assessments and best interest decision-making
- Contribute to Continuing Healthcare (CHC) processes and court work
- Build strong relationships with young adults, families, and multi-agency partners
Essential Requirements
- Social Work England Registration (essential)
- Enhanced DBS (in place)
- Minimum 5 years’ references / work history
- Right to work in the UK
- Experience in adult social care or transitions (desirable)
