Job Overview
We are seeking a passionate and experienced Care Manager to lead and oversee care services within our supported living in our clients homes in Plymouth, Devon, and surrounding areas of Cornwall. This role offers an exciting opportunity to make a meaningful difference by coordinating high-quality care tailored to each individual’s needs. You will be responsible for managing care teams, developing personalized care plans, and ensuring the delivery of compassionate, professional support.
Responsibilities
- Lead and supervise care staff to ensure the delivery of exceptional care and support
- Develop, implement, and review personalized care plans that meet each resident’s unique needs.
- Oversee medication administration processes in compliance with regulatory standards
- Manage daily operations within the supported living environment, ensuring safety and quality standards are maintained
- Provide leadership and guidance to your team through supervising experience, fostering a positive work environment
- Coordinate with healthcare professionals to support clients health and well-being
- Ensure compliance with legal requirements related to supported living regulations, and standards
Qualifications
- Proven experience in adult social care, supported living or a comparable environment.
- Previous experience as a Senior Support Worker or similar role with responsibility for staff and care quality.
- Demonstrated leadership skills with previous supervising or management experience
- Level 3/4/5 in health and social care / leadership qualification, or willingness to work towards one.
- Knowledge of care planning, medication administration, and regulatory compliance
- Excellent communication skills to liaise effectively with clients families, and healthcare professionals
- Autism-specific training or qualifications.
Pay: £36,000.00 per year
