Key Responsibilities
- Deliver tailored, compassionate care aligned with each individual’s personalised support plan.
- Assist adults with daily living activities, including nutrition, and household tasks.
- Provide emotional support and implement positive behaviour strategies to promote stability and wellbeing.
- Support individuals to access the community, attend appointments, or engage in meaningful activities.
- Maintain accurate, confidential records of care, observations, and any changes in health or behaviour.
- Work collaboratively with healthcare professionals, social workers, and other agencies to review and update care plans.
- Build strong, respectful communication with individuals, families, and external partners.
- Ensure a consistently safe environment by following all health and safety and safeguarding procedures.
Skills & Qualities
- Experience supporting adults with complex needs, disabilities, or long‑term health conditions is desirable but we have a full training programme available for prospective candidates where experience is lacking.
- Strong communication skills to engage confidently with individuals, families, and professional teams,
- Ability to follow detailed care plans while adapting to each person’s unique preferences and needs.
- We follow a sustainability plan and all our documentation is electronic, therefore, we will support all staff to become competent in using IT systems for documentation and reporting.
- Naturally compassionate, patient, and resilient, especially in challenging or sensitive situations.
- Organised and reliable, with the ability to manage multiple responsibilities effectively.
