In this role, you will:
- Identify social care issues on admission/ongoing and ensure that liaison with current housing providers, and Social Security Benefits are made, and that any issues regarding property and tenancy are dealt with at an early stage.
- Contribute to pre-admission assessments of patients referred to Forensic Services. This will include ascertaining information on Local Authority section 117 responsibility, and where applicable, identifying Nearest Relatives and appropriate family, determining previous input from mental health services and other relevant agencies and identifying any potential child protection or family issues.
- Function effectively across a variety of environments (e.g. wards of varying levels of security, service users’ homes, inter-agency settings, etc.) including contributing to home risk assessment
- Have a working knowledge of relevant legislation such as the Mental Health Act, Mental
Capacity Act and Care Act and to contribute to assessments of needs within the Care Act 2014 and Section 117 Aftercare needs, working with Local Authority and Health partners to ensure arrangements are made for effective discharge planning.
What you’ll need—and be able to provide evidence for:
· You will possess an active registration with Social Work England (SWE).
· You will have a recognised professional qualification in Social Work.
· You will have previous caseload management including complex conditions & cases
· You will be able to provide evidence based, reflective practice and clinical reasoning skills in assessment & treatment
· Desirable experience in Safeguarding processes and leadership structures
· You will have knowledge and application of relevant legislation eg Care Act, Mental Health Act, Mental Capacity Act
