We are seeking a passionate and experienced Care Coordinator to lead and manage care services within senior and Domiciliary care environments. In this vital role, you will oversee the development and implementation of personalized care plans, supervise care staff, and ensure the delivery of exceptional care that promotes independence, dignity, and well-being. Responsibilities
- Coordinate and oversee the creation, review, and updating of individual care plans and rota tailored to each resident’s needs.
- Supervise and support care staff, providing guidance, training, and leadership to ensure high standards of care delivery.
- Facilitate effective communication between residents, families, healthcare professionals, and team members to ensure seamless service provision.
- Conduct regular assessments of residents’ conditions and adjust care strategies accordingly to optimize health outcomes.
- Manage daily operations within the care environment, ensuring compliance with health and safety regulations.
- Organize transportation logistics for residents requiring outings or medical appointments.
- Maintain accurate documentation of care activities, incident reports, and progress notes using IT systems for record-keeping and reporting purposes.
- Attend Management meeting with the local authority and family members of our clients,
Experience
- Proven experience in senior or care home, or Domiciliary care settings with a strong background in supervising care staff.
- Demonstrated leadership abilities with a track record of managing or supervising teams effectively in reablement
- Experience developing and implementing comprehensive care plans tailored to individual needs.
- Excellent communication skills to liaise confidently with residents, families, healthcare providers, and team members.
- Familiarity with IT systems used for documentation, scheduling, and reporting in healthcare environments.
- Prior experience working within senior or elderly care settings is highly desirable.

Pay: £26,500.00-£28,000.00 per year
