Job Summary
We are seeking a compassionate and dedicated Health Care Assistant/Support Worker to join our team. In this role, you will provide essential support to individuals in care homes or within their own homes, assisting with daily activities and ensuring their well-being. Responsibilities
- Assist clients with personal care tasks such as bathing, dressing, and grooming
- Support individuals with mobility and daily activities to promote independence
- Follow care plans meticulously to meet each client’s specific needs
- Provide companionship and emotional support to service users
- Assist with medication administration as directed by healthcare professionals
- Maintain accurate records of care provided and any changes in condition
- Communicate effectively with clients, families, and healthcare teams to ensure coordinated care
- Support clients in participating in social activities and community engagement programmes
- Ensure a safe and clean environment in accordance with health and safety regulations
Skills
- Good communication skills in English, both verbal and written
- Ability to build rapport with clients and demonstrate empathy
- Basic IT skills for maintaining records and updating care plans
- Driving licence is desirable for supporting home care roles that involve travel between locations
- Experience working within a care home or home care setting is advantageous
- Knowledge of care plans and procedures relevant to health support work
- Organised, reliable, and able to work independently or as part of a team
