Overview
We're looking to bring on board an organised, enthusiastic, self-motivated and compassionate individual to join our care advice team and work closely with hospital discharge teams, commissioners and community services to deliver value-adding NHS & Social Care services. Based in Dorset and also covering neighbouring Counties, the core objective of the Care Advisor role at UBH will be to support individuals, their families and their representatives to find the right care, ensuring the best possible outcomes for the care seeker, in partnership with the care provider market and with the support of our health and social care partners. The role will require you to work alongside Health & Social Care Professionals, meeting key performance indicators that improve patient outcomes. Responsibilities
- Communicate effectively with clients, families, and healthcare providers to ensure clarity and understanding of care options.
- Provide information on available services, resources, and support within the community.
- Maintain accurate records of client interactions and updates to care plans using IT systems.
- Work closely with hospital discharge teams to identify individuals requiring support
- Work in partnership with care providers to source appropriate care
- Follow up with all relevant stakeholders to ensure a smooth, stress-free process
- Ability to work independently, with great attention to detail, and the ability to think proactively are essential
- Maintain accurate and up-to-date records on our CRM, liaising with health and social care staff to ensure that information is provided and updated where necessary.
- Monitor e-mail systems and exercise discretion to ensure that urgent and essential issues are brought to the team's attention. Proactively respond to and/or redirect other e-mails to relevant managers for action/information where appropriate.
- Build effective working relationships and work as part of a small team.
- Deal with interruptions and unplanned events as necessary, reprioritising work accordingly when required.
- To promptly refer any concerns about adult safeguarding or child protection to an appropriate person.
- Hybrid working from hospital sites and home with travel across Dorset and occasional travel outside of Dorset to other locations, including our Solihull headquarters.

- Proficiency in English, both written and verbal, is essential for effective communication.
- Strong administrative skills for managing documentation and client records efficiently.
- Focused, organised, self-driven, highly motivated and results-oriented
- Excellent interpersonal skills with a compassionate approach towards clients.
- Proven ability to work independently while also being a team player.
- Excellent written and oral presentation skills
- Empathetic
- Good working knowledge of Microsoft Office applications
- Driver with own car is essential - mileage and travel expenses will be paid
