Job Summary
We are seeking a dedicated and organised Clinical Care Co-ordinator to join our domicilliary care team. The successful candidate will play a vital role in managing patient care coordination, ensuring seamless communication between patients and healthcare providers, and supporting clinical operations. Duties
- Act as a liaison between clients, other healthcare professionals, and administrative staff to facilitate smooth communication.
- Maintain accurate and up-to-date patient records in compliance with data protection regulations.
- Assist with the preparation of clinical documentation and reports as required.
- Provide administrative support by managing other carers at work that day, answering queries, and coordinating with external agencies.
- Support the implementation of clinical protocols and procedures to enhance service delivery.
- Utilise office experience to manage scheduling systems, databases, and other administrative tools effectively.
- Contribute to continuous improvement initiatives within the clinical team.
Requirements
