- Monday to Friday 9:00am – 3:00pm- There will be occasions where you will need to work outside of this pattern.

Louisa Homecare Ltd is looking for a compassionate, organised and proactive Part-Time Care Coordinator to join our friendly office team.
This role is ideal for someone with experience in domiciliary care, community care or home care, who enjoys organising care services, supporting care staff and making a real difference to people's lives.
About the Role
As a Care Coordinator, you will play a vital role in ensuring our clients receive safe, high-quality, person-centred care.
Working from our office, you will coordinate care visits, support our Community Support Workers, communicate with healthcare professionals and ensure the smooth running of our daily care service.
You will use the Birdie Care Management System for:
- Care rostering
- Scheduling care visits
- Care planning
- Recording client information
- Communicating with care staff
Full Birdie training will be provided if required.
Key Responsibilities
- Coordinate daily domiciliary care visits using the Birdie system.
- Produce and manage staff rotas.
- Respond to changes throughout the day and arrange emergency cover where required.
- Answer telephone calls from carers, clients, family members and healthcare professionals.
- Support carers with operational queries and provide guidance where appropriate.
- Liaise with Social Workers, District Nurses, Occupational Therapists, GP surgeries and other healthcare professionals.
- Support quality assurance and compliance within the service.
- Participate in the on-call rota.
- Undertake community care visits when required to maintain continuity of care.

Essential Requirements
- Previous experience within Health & Social Care.
- Excellent communication and organisational skills.
- Confident using computer systems.
- Full UK driving licence and access to your own vehicle.
- Right to work in the UK.
Experience using Birdie or another care management system is desirable but not essential, as full training will be provided.
