At Axis Care, we are dedicated to enhancing lives by delivering high-quality care and support services. We are currently looking for compassionate, reliable, and experienced Outreach Support Workers to join our growing team.
This is an outreach role based in Manchester. You will provide person-centred support to service users within the local community, travelling between clients as required.
We welcome applications from both male and female support workers. Drivers and non-drivers are encouraged to apply, although a full UK driving licence and access to a vehicle may be advantageous for some placements.

Key Responsibilities
- Work hands-on in the community, supporting service users with a range of needs.
- Build positive relationships with service users, families, staff, and external professionals.
- Be proactive in problem-solving, responding to incidents, and escalating concerns appropriately.
- Attend training and contribute to our continuous quality improvement approach.
- Maintain accurate records and follow care plans and risk assessments.
- Promote independence, dignity, and wellbeing while delivering high-quality person-centred care.
Requirements
- Previous experience in social care (ideally in domiciliary care, outreach, or supported living settings).
- A caring, confident individual with a strong team ethic.
- Excellent verbal and written communication skills.
- A full UK driving licence and access to a vehicle is desirable but not essential.
- Willingness to complete or currently be working towards a Level 3 Diploma in Health & Social Care.
- Able to use initiative and remain calm under pressure.
- Passionate about developing a career in care.
