Join our dedicated care team as a Care Home Assistant Practitioner, where your passion for senior care and leadership skills will make a meaningful difference in residents’ lives. This role offers an exciting opportunity to work in a supportive environment, working under the direction of the nominated Nurse in Charge to provide medical care, support clinical decisions and to undertake the safe administration of medication whilst focusing on delivering high-quality, person-centered care. As a key member of our team, you will supervise care staff, develop and implement care plans, and ensure the well-being and dignity of every resident. Your proactive approach and excellent communication skills will help foster a warm, respectful atmosphere that promotes independence and comfort.
Responsibilities
- Supervise and support care staff to deliver compassionate, personalised care to residents in accordance with their individual needs and care plans
- Assist in the development, review, and implementation of comprehensive care plans tailored to each resident’s health requirements
- Communicate effectively with residents, families, healthcare professionals, and team members to ensure seamless coordination of care
- Lead by example in maintaining high standards of hygiene, safety, and resident dignity across all activities
- Support residents with daily activities such as mobility, medication management, and personal hygiene while promoting independence
- Contribute to the training and mentoring of junior staff to enhance team performance and professional growth
- Utilize IT systems to document resident progress, update care plans, and report incidents accurately and promptly
Experience
- Previous supervising or leadership experience within a care home or senior care setting is essential
- NVQ level 3 in Health and Social Care
- Proven ability to develop and manage care plans effectively while adhering to regulatory standards
- Strong communication skills to liaise confidently with residents, families, healthcare teams, and colleagues
- Experience in using IT systems for documentation and reporting purposes in a healthcare environment
- Demonstrated understanding of safeguarding procedures and compliance with health & safety regulations

Pay: £14.65 per hour
