Job Purpose
To provide proactive, person-centred housing support to individuals moving on from supported and temporary accommodation into suitable, sustainable long-term housing.
The post holder will work with people who may have complex needs or multiple barriers to securing and maintaining accommodation, providing practical advice, advocacy and support to promote independence, prevent repeat homelessness and enable successful tenancy sustainment.
- Support individuals to move from supported and temporary accommodation into suitable long-term housing.
- Complete housing support assessments, risk assessments and person-centred support plans.
- Develop and implement effective move-on and resettlement plans.
- Provide advice and practical support on housing options, applications, tenancy responsibilities and housing processes.
- Support service users with budgeting, welfare benefits, debt management and income maximisation.
- Work collaboratively with housing providers, health professionals, support services and other partner agencies.
- Advocate on behalf of service users and facilitate access to appropriate services and community resources.
- Support nominations, referrals and housing pathway processes.
- Identify and address barriers to successful move-on and tenancy sustainment.
- Provide guidance and support to colleagues, providers and partner agencies on move-on and resettlement practices where required.
- Maintain accurate case records, assessments, support plans, reports and outcome information.
- Monitor progress and review support plans in response to changing needs.
- Manage complex situations appropriately and take action to safeguard service users where required.
- Promote independence, choice and positive housing outcomes.
- Work in accordance with relevant legislation, policies, procedures and safeguarding requirements.
- Experience of working with vulnerable individuals or groups.
- Experience providing housing-related support and/or assessing housing needs.
- Knowledge and understanding of housing, homelessness or supported accommodation services.
- Strong communication and interpersonal skills.
- Ability to build positive and professional relationships with service users.
- Excellent organisational and time-management skills.
- Ability to manage a caseload and prioritise competing demands.
- Ability to assess complex situations, identify risks and make sound decisions.
- Experience working collaboratively with multiple agencies and stakeholders.
- A-Level qualification or equivalent, or substantial relevant experience.
- Ability to maintain accurate and appropriate case records.
- Commitment to promoting independence, tenancy sustainment and positive outcomes.
- Experience working within homelessness, housing support or supported accommodation services.
- Knowledge of Welsh housing legislation and homelessness services.
- Relevant housing, homelessness or supported housing qualification.
- Welsh language skills.
- Experience supporting individuals with complex or multiple needs.
Hours per Week: 37 hours per week
Location: Newport, South Wales
Bi-Weekly Payments: Payment will be made on a bi-weekly basis.
