Job Summary
We are seeking a experienced, dedicated and organised Care Coordinator to join our team. The successful candidate will play a vital role in coordinating care services, ensuring seamless communication between patients, families, and healthcare professionals. Prior NHS GP practice experience is essential in a receptionist or care coordinator role.
Responsibilities
The candidate must be competent at the online triage model for NHS general practice, and be familiar with Accurx or similar online tools.
Be able to implement basic online patient triage and navigate patient care to appropriate appointments ,and services.
- Coordinate care plans and services tailored to individual client needs, ensuring timely delivery of support.
- Act as a primary point of contact for patients,families, and healthcare providers, maintaining clear and compassionate communication.
- Schedule appointments, manage documentation, and update client records accurately using organisational software.
- Liaise with healthcare professionals to facilitate service delivery and address any issues or concerns promptly.
- Assist with administrative tasks such as filing, data entry, and correspondence to maintain smooth office operations.
- Monitor client progress and adjust care plans in collaboration with relevant professionals as required.
- Ensure compliance with organisational policies and procedures related to confidentiality and data protection.
Qualifications
- Proven experience working in a NHS GP environment, demonstrating strong organisational skills.
- Excellent communication skills, both written and verbal, with a compassionate approach towards clients.
- Ability to prioritise tasks effectively and manage time efficiently in a busy setting.
- Proficiency in using Emis Web, System one,and Accurx tools.
- A proactive attitude with the ability to work independently as well as part of a team.
