Access Your Care is an established care provider, working directly for North Somerset Council as their leading domiciliary care provider. We are excited to announce that we are growing our service planning team.
What is a Service Planner?
Service planners schedule our clients calls, make sure that our service runs smoothly, ensure that all of our clients receive the care they require and support our team in the field. As a planner, your main responsibilities will include:
- Ensuring that all of our clients have the correct appointments booked in line with their care plans and needs.
- Communicating with clients and carers to update of any changes in their runs/call times.
- Planning carers daily workload ensuring that their run is achievable and within their working hours.
- Working with our coordinators to ensure all new clients have a smooth start to their service.
- You will also be responsible for answering the phones, assisting with client and staff queries.
- Following up on any concerns that are reported during working hours and out of hours.
- You will also be part of our 8 week on-call manager rota which is office-based. This is comprised of weekday evenings (17:00-22:30) and weekend shifts (07:00-22:30).
About you:
You will have a high level of organisation, have an ability to work well under pressure and have experience working with multiple internal and external stakeholders simultaneously.
You may also be delivering hands on care when required.

A rota you can rely on
