THE OPPORTUNITY
We are seeking an experienced, motivated and clinically skilled Lead Nurse or clinical nurse specialist.
to join our team at John Wilson House. This is a pivotal role within our service, providing clinical
leadership, operational oversight, and governance across our specialist supported living provision.

full-time applicants and job-share arrangements between two Lead Nurses.

JOB PURPOSE
The Lead Nurse is responsible for providing clinical leadership and operational oversight at John Wilson House, ensuring the delivery of safe, effective and person-centred care to adults with complex physical health needs, learning disabilities and autism. The post holder will uphold the highest standards of clinical governance, staff competency and quality assurance in line with CQC Fundamental Standards, the NMC Code, NICE guidance and Partnership Care & Support Ltd policies and procedures.
The Lead Nurse will be the clinical backbone of the service – supporting the Registered Manager in all aspects of clinical quality, leading the nursing team, developing staff skills, and acting as the primary clinical point of contact for commissioners, families and external professionals.
MAIN RESPONSIBILITIES
1. Clinical Leadership
- Provide day-to-day clinical leadership across John Wilson House.
- Lead the nursing team in delivering evidence-based, person-centred care to all service users.
- Ensure safe, well-reasoned clinical decision-making across all nursing interventions.
- Support complex clinical assessments, including health needs assessments and care planning.
- Lead multidisciplinary team (MDT) meetings as appropriate, acting as the clinical lead voice.
- Monitor the health and wellbeing of all service users and identify early signs of deterioration.
- Ensure timely and appropriate escalation of deteriorating or unstable health conditions.
- Promote a culture of clinical openness, safety and continuous improvement.
2. Clinical Care Oversight
The Lead Nurse will maintain oversight of and set the standard for care delivery across the following clinical areas:
- PEG / PEG-J enteral feeding and nasojejunal (NJ) feeding management.
- Complex medication management, including high-risk medications and controlled drugs.
- Catheter care and suprapubic catheter management.
- Respiratory care, including non-invasive ventilation (BiPAP/CPAP).
- Oxygen therapy and airway management.
- Oral and oropharyngeal suctioning.
- Pressure area care and wound management.
- Epilepsy management including rescue medication (buccal midazolam) protocols.
- Continence care and bowel management.
- Diabetes management where applicable.
- Infection prevention and control.
- Moving and handling for service users with complex physical needs.
- End-of-life care planning and palliative support where applicable.
- Clinical observations and vital signs monitoring.
3. Clinical Governance
- Lead clinical audits and ensure timely completion of audit cycles.
- Investigate incidents, near misses and medication errors, completing Root Cause Analysis investigations where required.
- Promote and embed the Duty of Candour across the nursing team.
- Implement lessons learned from incidents, complaints and clinical reviews.
- Monitor compliance with all clinical policies and procedures.
- Maintain governance dashboards and contribute to quality reporting.
- Ensure safe, accurate and contemporaneous clinical record-keeping.
- Monitor and maintain staff competency records and training compliance.
4. Leadership and Management
- Lead, motivate and professionally develop the nursing team.
- Support the Team Lead and Care Coordinator in their roles.
- Promote effective, collaborative and respectful multidisciplinary working.
- Participate in the recruitment, selection and induction of clinical staff.
- Manage clinical performance, including addressing underperformance appropriately.
- Conduct annual appraisals and contribute to professional development planning.
- Provide regular clinical supervision and reflective practice sessions.
- Produce and manage nursing rotas, ensuring safe staffing levels at all times.
- Monitor sickness and absence, authorising leave within delegated authority.
- Coordinate emergency cover and ensure continuity of care.

6. Stakeholder Engagement and Quality Assurance
- Liaise with commissioners, ICBs, clinical professionals, families and external agencies.
- Attend safeguarding meetings, MDT meetings and commissioner reviews as the clinical lead.
- Ensure CQC compliance across the five key domains: Safe, Effective, Caring, Responsive, and Well-Led.
- Contribute to inspection readiness: maintaining current care plans, risk assessments, competency records and audit schedules.
- Monitor and report on clinical KPIs, including medication audit outcomes, incident trends, infection control compliance, safeguarding actions and hospital admissions.
- Maintain and review Health Action Plans, Hospital Passports and service improvement plans.
- Lead or contribute to clinical quality improvement projects.
PERSON SPECIFICATION

QUALIFICATIONS AND REGISTRATION
Registered Nurse (Adult – RN) and/or Registered Nurse (Learning Disability – RNLD).
Current, valid NMC registration with no conditions or restrictions.
Dual NMC registration (Adult and Learning Disability).
Relevant post-registered clinical qualification or specialist practitioner qualification.
Teaching and assessing/mentorship qualification (e.g., ENB 998, PGCE, Practice Assessor training).
Leadership or management qualification.
EXPERIENCE
Minimum 5 years post-registration nursing experience.
Demonstrated experience supporting adults with complex physical health needs.
Experience working within learning disability and/or autism services.
Experience in supported living, residential care or community nursing.
Experience in a clinical leadership, senior or charge nurse role.
Experience conducting clinical audits and root-cause analysis investigations.
Experience in competency assessment and sign-off of clinical staff.
Experience managing PEG/PEG-J/enteral feeding.
Experience managing non-invasive ventilation (BiPAP/CPAP) in a community or supported living setting.
Experience of end-of-life care and palliative care planning.
Experience in DNACPR discussions and documentation.

TERMS AND CONDITIONS OF EMPLOYMENT
Contract Type: Permanent, subject to a satisfactory six-month probationary period.
Working hours: 37.5–40 hours per week. Full-time or job-share arrangement between two Lead Nurses (subject to agreement).
Probation: six months. Performance, conduct and attendance will be assessed during this period.
Salary: Competitive and commensurate with experience. Details provided upon application.

Training: Mandatory participation in all required clinical and statutory / mandatory training is a condition of employment.
On-Call: Participation in the clinical on-call rota is required. On-call arrangements will be confirmed on appointment.

Safeguarding: The post holder is responsible for promoting and safeguarding the welfare of all service users. All safeguarding obligations and policies must be complied with.
Flexibility: The Lead Nurse may be required to work across Partnership Care & Support Ltd services where clinically appropriate and in agreed circumstances.
Accountability: The Lead Nurse is professionally accountable under the NMC Code and operationally accountable to the Registered Manager and Director.
