Job Summary
We are seeking two dedicated, experienced and professional Care Coordinators to support the safe, effective and well-led running of our residential care home.
The Care Coordinator will work under the direction of the Facilities Manager and Home Management Team and will be responsible for supervising and supporting Team Leaders, Senior Care Assistants and Health Care Assistants across the home. This role is similar to a Unit Manager / Floor Coordinator position and requires strong leadership, excellent care knowledge, good medication oversight, and confidence in managing care-related documentation.
The successful candidate will play a key role in ensuring residents receive safe, person-centred and dignified care in line with individual care plans, risk assessments, mental capacity requirements, medication procedures and regulatory expectations. The role will involve a mixture of day and night shifts, arranged flexibly according to the needs of the service.

Responsibilities
- Support the Facilities Manager and Home Management Team with the daily operational running of the residential care home.
- Supervise, guide and support Team Leaders, Senior Care Assistants and Health Care Assistants to ensure care is delivered safely, professionally and in line with residents’ assessed needs.
- Oversee the delivery of person-centred care, promoting residents’ dignity, independence, wellbeing, privacy and choice at all times.
- Support the development, implementation and regular review of personalised care plans and risk assessments, including mobility, falls, nutrition and hydration, skin integrity, continence, behaviour, communication, medication and dementia-related needs.
- Complete and support care-related documentation, including care plans, risk assessments, MCA assessments, best interest documentation, daily notes, incident forms, body maps, ABC charts and monitoring records.
- Provide medication oversight, including medication administration where required, MAR chart checks, medication audits, stock checks, PRN protocols and escalation of medication concerns.
- Liaise professionally with GPs, District Nurses, pharmacies, social workers, hospitals, families and other external professionals to ensure residents receive timely and appropriate support.
- Monitor residents’ health and wellbeing, identify any changes or concerns, and escalate appropriately to management, GP, District Nurse or emergency services where required.
- Support audits and quality assurance checks across the home, including medication audits, care plan audits, infection prevention checks, documentation audits and general care compliance monitoring.
- Assist with staff supervision, coaching, competency checks and performance monitoring, escalating any practice concerns to the Facilities Manager or Home Management Team.
- Ensure staff complete required care tasks, observations, monitoring charts and daily records accurately and in a timely manner.
- Support effective handovers between shifts to ensure all staff are aware of resident needs, risks, appointments, professional input and changes in care.
- Promote safe moving and handling, infection prevention and control, safeguarding, dignity, confidentiality and person-centred care.
- Ensure the care environment remains safe, organised and responsive to residents’ needs.
- Work flexibly across mixed shifts, including days, nights, weekends and bank holidays, according to the needs of the home.
Skills and Experience
- Previous experience in a residential care home, dementia care home, nursing home or similar health and social care setting.
- Experience working as a Senior Care Assistant, Team Leader, Care Coordinator, Unit Manager or similar senior care role.
- NVQ Level 3 in Health and Social Care or relevant diploma/equivalent qualification is required.
- Strong knowledge of care planning, risk assessments and care documentation.
- Good understanding of MCA, best interest decision-making, DoLS principles and safeguarding procedures.
- Strong medication administration knowledge, including MAR charts, PRN medication, medication audits and safe medication procedures.
- Good computer skills and confidence using electronic care planning systems.
- Strong verbal and written communication skills.
- Ability to liaise confidently and professionally with residents, families, staff, GPs, pharmacies, District Nurses, social workers and other professionals.
- Ability to supervise staff, delegate tasks appropriately and support a positive team culture.
- Excellent organisational skills with the ability to prioritise during busy shifts.
- Good understanding of CQC expectations, person-centred care, dignity, privacy, safeguarding, infection control and health and safety.

Essential Requirements
- NVQ Level 3 in Health and Social Care or relevant diploma.
- Senior care / leadership experience in a care home setting.
- Medication administration experience.
- Medication audit or MAR chart checking experience.
- Care plan and risk assessment experience.
- Good computer and documentation skills.
- Strong written and verbal communication.
- Ability to work flexible mixed shifts, including days and nights.
- Right to work in the United Kingdom.
Pay: £14.21-£14.95 per hour
