Main duties of the job
In this role, you will not only perform routine nursing and clinical tasks related to patient care, but you will also lead and coordinate care for patients with long-term conditions (e.g., diabetes, COPD, asthma, hypertension).

Job responsibilities
Job Title: Practice Nurse
Hours of work: 22hrs per week
Pay dependent on experience.
Assist in and perform routine general nursing services and administrative tasks to deliver excellent patient care, including:
- Coordinating and delivering Chronic Disease Management - Hypertension Management, Diabetes, COPD / Asthma
- Cervical Cytology
- Wound care / ulcer care / Doppler
- Removal of Sutures
- New Patient Medicals/Urinalysis
- Travel Clinics
- ECGs
- Venepuncture
- Peak Flow Readings / Spirometry
- Routine immunisations/Childhood immunisations
- Chaperoning and assisting patients where appropriate who are being examined by another clinician
- Requesting pathology tests, for example urine culture, swabs
- Health promotion.
- Other tasks will include:
- Maintaining and cleaning equipment used by the nurses and GPs
- Maintaining GP and Nurses rooms, stocking and rotating items as required
- Maintain general tidiness and cleanliness of nurses and treatment rooms
- Participation in administrative systems in the practice
- Attend and participate in any meetings when required.
- On occasion there may be a requirement to undertake home visits
- Any other delegated duties appropriate to the post

- Professional responsibilities
- Work in accordance with NMC The Code Professional Standards of Behaviour and Practice for Nurses and Midwives
- To demonstrate safe and effective practice by meeting the standards set out in the NMC Code and evidence of complying with the necessary revalidation requirements for registration three yearly.
- To ensure that professional and clinical practice is delivered safely, effectively, and with quality and compassion in accordance with the Care Quality Commissions regulations for General Practice
- To work in a manner that is always respectful of the confidentiality issues relating to patients both directly and indirectly.
- You will be expected to produce work to the highest standard to provide quality at all times in a professional manner in line with clinical governance arrangements.
Person SpecificationExperienceEssential
- Experience of working as a practice nurse in a GP practice.
- Experience of managing chronic conditions in a primary care setting
- Involvement in implementing and using protocols and clinical guidelines
- Experience in wound management, leg ulcers, cervical cytology, and immunisations and vaccinations.
- Experience of using clinical systems and Microsoft office

- Experience of working with stakeholders to demonstrate collaborative working
Qualifications
Essential
- Registered Nurse
- Accredited qualification in Diabetes care
- Accredited qualification in Asthma/COPD
