Band 6 Specialist Physiotherapist – Community Rapid Response
Location: Redbridge, London (covering Redbridge, Havering & Barking and Dagenham)
Salary: £27 per hour
Contract: Ongoing
Hours: Full-time – 37.5 hours per week (7-day service with working hours agreed based on service demand)
Sanctuary Personnel is seeking an experienced Band 6 Specialist Physiotherapist to join a dynamic Community Rapid Response team in Redbridge. This is an exciting opportunity to work within an urgent community service focused on preventing unnecessary hospital admissions, supporting early discharge, and delivering high-quality rehabilitation in patients' own homes.
Working as part of a multidisciplinary team, you will provide rapid physiotherapy assessment and intervention for patients experiencing acute functional decline, helping them remain safe and independent within the community.
Key Responsibilities
- Undertake urgent physiotherapy assessments for patients presenting with acute mobility or functional deterioration in community settings.
- Deliver evidence-based rehabilitation programmes aimed at preventing hospital admission and promoting recovery at home.
- Assess mobility, balance, transfers, strength, falls risk, and functional ability.
- Develop and implement individualised treatment plans to maximise independence and improve patient outcomes.
- Recommend and arrange appropriate mobility aids, equipment, and environmental adaptations.
- Support early supported discharge pathways and help prevent avoidable hospital readmissions.
- Provide education, advice, and self-management strategies to patients, families, and carers.
- Work collaboratively with multidisciplinary teams, including nurses, occupational therapists, paramedics, GPs, and social care professionals.
- Maintain accurate, timely, and professional clinical records in accordance with HCPC and local standards.
- Contribute to service development, quality improvement initiatives, and achievement of service performance targets.
Requirements
- Degree or equivalent qualification in Physiotherapy.
- Current HCPC registration as a Physiotherapist.
- Recent NHS experience within a Physiotherapy setting.
- Post-qualification experience working as a Physiotherapist.
- Community Physiotherapy experience.
- Experience managing urgent, complex, or frail patient caseloads.
- Experience completing falls assessments and delivering rehabilitation programmes.
- Excellent clinical reasoning, assessment, and decision-making skills.
- Ability to work autonomously while contributing effectively within a multidisciplinary team.
- Excellent communication and documentation skills.
