About the Role
As Care Coordinator, you will be responsible for coordinating care services, managing staff rotas, responding to changes in client needs and ensuring continuity of care. You will work closely with the Registered Manager, care staff, clients, families and healthcare professionals to ensure the highest standards of care are maintained.
Key Responsibilities
- Produce and manage efficient staff rotas using electronic care planning systems.
- Allocate care visits while ensuring continuity of care wherever possible.
- Respond promptly to sickness, annual leave and emergency staffing shortages.
- Ensure all care visits are covered safely and effectively.
- Liaise with clients, their families and healthcare professionals regarding care arrangements.
- Complete care reviews and assist with care assessments where required.
- Update care plans and ensure all documentation remains accurate and compliant.
- Monitor staff attendance, punctuality and visit completion.
- Maintain regular communication with care staff through telephone, email and WhatsApp.
- Respond to out-of-hours issues when required as part of the management on-call rota.
- Support the recruitment, induction and onboarding of new care staff.
- Ensure compliance with CQC regulations, safeguarding procedures and company policies.
- Assist with incident reporting, complaints and quality assurance processes.
- Maintain accurate records using the company's digital care management system.
- Build positive relationships with commissioners, social workers, district nurses, GP surgeries and other healthcare professionals.
- Support the Registered Manager in the day-to-day running of the service.
- Essential Requirements
- Previous experience working as a Care Coordinator within domiciliary care.
Desirable
- Experience using PASS, CM2000 or other electronic care management systems.
- NVQ Level 3 in Health & Social Care (or equivalent).
- Experience completing care assessments and reviews.
- Knowledge of safeguarding adults and Mental Capacity Act.
- Experience working with local authority and NHS referrals.

Pay: £30,000.00-£32,000.00 per year
