Salary: £26,000 – £27,000 (dependent on experience)
Hours: Full-time (including participation in an on-call rota)

The Role
We are looking for an experienced Care Coordinator who thrives in a fast-paced environment and can confidently manage multiple priorities. You will coordinate care packages, manage staff rotas, and ensure our clients receive a consistent and high standard of care.
Key Responsibilities
- Plan and manage staff rotas to ensure all care visits are covered
- Coordinate care packages and maintain continuity of care
- Liaise with clients, families, and care staff
- Carry out assessments, care plan reviews, and updates
- Respond to rota changes, sickness, and emergencies
- Support compliance with CQC standards
- Participate in the on-call rota
What We’re Looking For
- Previous Care Coordinator experience (essential)
- Background in domiciliary care
- Strong organisational and communication skills
- Ability to multitask and work under pressure
- Good understanding of CQC standards
