We are seeking a compassionate and dedicated Social Care Workers to join our team. Responsibilities
- Provide personalised support to clients in accordance with their care plans, ensuring their physical, emotional, and social needs are met effectively.
- Maintain clear and open communication with clients, families, and team members to facilitate coordinated care delivery.
- Develop, review, and update individualised care plans in collaboration with relevant professionals and the clients themselves.
- Assist clients with daily activities such as personal hygiene, meal preparation, medication management, and mobility support.
- Promote independence by encouraging clients to participate in community activities and social engagement opportunities.
- Record detailed notes on client progress, incidents, and any changes in circumstances using IT systems accurately and promptly.
- Lead or participate in team meetings to discuss client progress, share insights, and contribute to continuous improvement initiatives.
- Uphold confidentiality and adhere strictly to safeguarding policies at all times.
Qualifications
- Strong communication skills with the ability to build trusting relationships with clients from diverse backgrounds.
- Proficiency in IT systems for record keeping, reporting, and communication purposes.
- Demonstrated leadership qualities with the ability to motivate both clients and colleagues positively.
- Experience or knowledge of developing and implementing care plans tailored to individual needs is desirable.
- Compassionate attitude with a genuine desire to support vulnerable individuals towards greater independence.
- Relevant qualifications or training in social care or related fields are advantageous but not essential; willingness to undertake necessary training is expected.
