We are looking for a caring, fun and confident Support Worker to join a dedicated team supporting a child in Chatham.

The Role
As part of a small, dedicated care team, you will support our client before and after school, helping her maintain her daily routines and participate fully in community life.
Your responsibilities will include:
- Supporting with morning and after-school routines
- Assisting with personal care, dressing and daily living activities
- Preparing and supporting with meals and hydration
- Administering prescribed and PRN medication in line with care plans and training
- Driving and accompanying our client to and from school using her automatic wheelchair accessible vehicle (WAV)
- Supporting attendance at therapies, activities, appointments and family visits
- Encouraging independence while ensuring safety and emotional wellbeing
- Helping our client engage positively with her therapy programmes and developmental goals
- Supporting our client to undertake educational activities including homework.
- Working closely with family members and the wider multidisciplinary team (MDT)
- Confidently contributing to MDT meetings and providing feedback on progress, wellbeing and outcomes
- Supporting our client through upcoming transitions and life changes with patience, consistency and encouragement
Clinical Skills Required
- Medication administration, including PRN medication
- Supporting eating and drinking in line with care plans
- Moving and handling
- Understanding and following therapy programmes
- Recognising and responding appropriately to changes in physical or emotional wellbeing

What You'll Need
- A minimum of 6 months' care experience
- Full UK driving licence (automatic accepted)
- Full right to work in the UK
- Communication and IT skills
- Understanding of safeguarding principles
- Ability to recognise deterioration in both physical and emotional wellbeing
- A commitment to delivering high-quality, family-centred care
