Overview
We're looking for a Domiciliary Care Assessor who knows what good care actually looks like ---because you'll be the one shaping it before a single visit takes place.
You'll sit down with people in their own homes, often at a vulnerable moment, and work out what support actually looks like for them: what they can still do, what they need help with, where the risks are, and what matters to them beyond just the practical stuff. From there, you'll build a care plan that care workers can actually follow -- clear enough to keep someone safe, flexible enough to respect how they want to live.
You won't be doing this alone. You'll be talking to families who are worried, GPs and social workers who need updates, and care staff who need to understand a person's needs in five minutes flat before their first visit. Getting that communication right is half the job.
We need someone who's done this before -- who understands CQC expectations not as a checklist but as the standard behind good practice, who can spot a risk before it becomes an incident, and who genuinely believes people deserve to stay independent and in control of their own lives for as long as possible.

Responsibilities
- Conduct comprehensive assessments of service users' care, health, wellbeing, personal, and social support needs.
- Complete initial assessments, reviews, and reassessments to ensure care packages remain appropriate and responsive to changing needs.
- Develop person centred care plans in partnership with service users, families, care staff, and healthcare professionals, ensuring individual preferences, routines, outcomes, and goals are reflected.
- Complete risk assessments relating to mobility, falls, medication, moving and handling, nutrition, safeguarding, infection prevention, and home environment safety.
- Use electronic care planning systems to maintain accurate, timely, and confidential records in line with GDPR and CQC requirements.
- Liaise with social workers, GPs, district nurses, occupational therapists, and other professionals to support coordinated care delivery.
- Communicate care plans, risks, and individual support requirements clearly to care staff before care services commence.
- Monitor changes in service users' needs and update care plans and risk assessments following reviews or changes in circumstances.
- Identify and escalate safeguarding concerns, incidents, complaints, and risks in line with organisational procedures.
- Maintain accurate documentation that supports compliance with CQC Fundamental Standards and quality assurance requirements.
- Support care quality monitoring through reviews, audits, feedback, and continuous improvement activities.
- The Care Assessor will participate in the on-call rota as required, typically on a two-week rotation, to support continuity of care and respond to service needs.
Skills
- Proven experience in domiciliary care, community care, care assessment, care coordination, or a similar health and social care role.
- Experience completing care needs assessments, risk assessments, care plan reviews, and personalised support plans.
- Strong knowledge of CQC Fundamental Standards, safeguarding procedures, the Care Act 2014, and the Mental Capacity Act 2005.
- Understanding of person centred care principles, promoting independence, dignity, choice, privacy, and wellbeing.
- Ability to identify individual care needs, assess risks, and develop safe and effective care solutions.
- Experience using electronic care planning systems and maintaining accurate, confidential service user records.
- Excellent communication skills with the ability to engage positively with service users, families, care workers, and healthcare professionals.
- Ability to work collaboratively with multidisciplinary teams, including social workers, GPs, district nurses, occupational therapists, and other professionals.
- Strong written and verbal communication skills, with the ability to produce clear assessments, care plans, and reports.
- Good understanding of safeguarding, medication support, moving and handling, infection prevention, and risk management within domiciliary care.
- Strong organisational skills with the ability to manage multiple assessments, reviews, and priorities effectively.
- Ability to work independently, demonstrate professional judgement, and respond appropriately to changes in service users' needs.
