An exciting opportunity has arisen for a motivated Diabetes Specialist Nurse to join our dynamic multidisciplinary team in Herefordshire.
We welcome applications from nurses with experience in diabetes and those looking to develop their career in this specialty. For nurses without previous diabetes specialist experience, we offer a supported Band 5–6 development pathway , including a tailored induction, dedicated development time and support to achieve TREND-UK-aligned competencies, with progression to Band 6 as competencies are achieved.
This varied 30-hour role spans inpatient and outpatient settings, with opportunities to work in subspecialty diabetes clinics. You will provide individualised care, support self-management and shared decision-making, deliver structured education to patients and healthcare professionals, and contribute to preventing avoidable admissions.
You will work autonomously and as part of a multidisciplinary team, using evidence-based practice and contributing to quality improvement and service development.

The successful candidate will:
- Provide high-quality, holistic and individualised care to people with diabetes.
- Work collaboratively with people with diabetes, their families and carers where appropriate.
- Support self-management, shared decision-making and patient empowerment.
- Provide diabetes specialist nursing support across acute and inpatient settings, helping to prevent avoidable hospital admissions and facilitate safe, effective discharge.
- Contribute to outpatient and subspecialty diabetes clinics.
a Assess, initiate and provide ongoing support of diabetes technologies including CGM and insulin pump therapy.
- Deliver structured education to people with diabetes and education to healthcare professionals.
- Work autonomously as well as alongside senior diabetes specialist nurses.
- Use evidence-based practice to assess, plan, implement and evaluate care.
- Contribute to quality improvement and ongoing service development.
