The Role
As a Care Coordinator, you will be responsible for coordinating care packages, managing staff rotas, and ensuring that our clients receive outstanding care that meets their individual needs.
You will act as the key link between clients, care staff, families, and healthcare professionals, ensuring our services are delivered safely and effectively.
Key Responsibilities
Coordinate and schedule care visits efficiently using our rostering system.
Match carers to clients based on skills, experience, and client preferences.
Ensure all care visits are covered and respond quickly to any changes or emergencies.
Liaise with clients, families, social workers, and healthcare professionals.
Support the onboarding of new care packages and conduct care reviews.
Maintain accurate client and staff records in line with CQC requirements.
Support the recruitment, induction, and supervision of care staff.
Monitor staff availability, annual leave, and training compliance.
Participate in the on-call rota as required.
Ensure all services are delivered in accordance with company policies and CQC standards.
Be able to stand in for Carers during emergencies
What We're Looking For
Essential:
Previous experience as a Care Coordinator, Scheduler, or Senior Carer within domiciliary care.
Good understanding of CQC regulations and person-centred care.
Excellent organisational and communication skills.
Ability to work under pressure and manage competing priorities.
Good IT skills and experience using care management systems.
Full UK driving licence and access to a vehicle.
Right to work in the UK.
Desirable:
NVQ Level 3 in Health & Social Care or equivalent.
Experience using systems such as:
Birdie
Access Care Planning
PASS
CM2000
People Planner
Experience managing private and local authority-funded care packages.
