The Growth Company’s (GC) Employability Service within Manchester Foundation Trust (MFT) is seeking a Work and Health Hub Guide to support delivery within our MFT Health and Wellbeing Hubs. The successful candidate will promote the service across MFT sites, build relationships with hospital departments and partner organisations, generate referrals, complete triage and eligibility calls, and support participants through the initial stages of their employability journey.
At GC, we deliver Employability Services in partnership with Manchester University NHS Foundation Trust (MFT). Through this service, we support people with disabilities and health conditions to find, stay in and succeed in work through tailored, person-centred employment support. Working closely with individuals, employers and healthcare professionals, we help overcome barriers to employment, identify workplace solutions and create positive, sustainable outcomes that improve wellbeing and career prospects.
Key Responsibilities:
- Raise awareness of the Employability Service across MFT through engagement activities, presentations, drop-ins, events and awareness campaigns to increase referrals into the service.
- Work to monthly set targets for referrals, attaching new participants to the programme who will continue engagement.
- Work in partnership with Health Hub teams, Integration Coordinators and Work and Health Coaches to achieve referral and engagement targets.
- Develop and implement a range of referral-generation activities, including telephone outreach, face-to-face engagement, follow-up calls, networking events and roadshows.
- Conduct triage calls and initial conversations with potential participants to identify their needs and suitability for support, completing all required onboarding activity.
- Refer people to relevant sources of further advice and support.
- Maintain effective communication with Work and Health Coaches and wider service teams to ensure seamless participant handovers.
- Complete initial assessment paperwork, including documentation to check eligibility for the programme and required initial assessments.
About You:
- Experience of working with a range of organisations and stakeholders to promote services and generate referrals.
- Ability to organise and coordinate promotional activities, awareness events and engagement sessions.
- Understanding of the barriers faced by individuals with health conditions, disabilities or other challenges that may impact employment.
- Experience of building productive working relationships with healthcare professionals, community organisations or partner agencies.
- Strong communication and interpersonal skills, with the ability to build rapport quickly.
- Ability to manage a varied workload and work effectively within a target-driven environment.

- Excellent communication, engagement and relationship-building skills, with a professional and confident telephone manner.
- Strong organisational and time-management skills, with the ability to manage a varied workload.
- Confident working towards challenging targets and key performance indicators.
- Flexible approach to work, including occasional travel across MFT sites and community venues.
- Able to work independently and as part of a multidisciplinary team.
- Desirable experience within healthcare, wellbeing, employability or community services, or the ability to speak a community language relevant to Greater Manchester.
