Working Hours: Full time – 37.5 hours per week (core hours between 08:00–18:30)

We are recruiting a Care Coordinator to join our friendly and growing Primary Care Network (PCN) team. This is an opportunity to play a vital role in improving outcomes for patients, particularly those who are vulnerable or living in care and nursing homes.
About the role
As a Care Coordinator, you will work as part of a supportive multidisciplinary team, helping patients receive coordinated, personalised care that reflects what matters most to them.
You will manage a defined caseload, working closely with patients, carers, GP practices, care homes and community services to ensure care is well-organised, proactive and responsive.
This is a varied, people‑focused role combining patient contact, coordination, advocacy and administration — ideal for someone who enjoys making a difference while developing their skills in primary care.
What you’ll be doing
- Supporting patients to navigate health, social care and community services
- Acting as a key point of contact for patients, carers and care homes
- Developing personalised care and support plans
- Coordinating appointments, referrals and communication across services
- Working closely with GPs, social prescribers and wider PCN colleagues
- Helping patients access appropriate support, benefits and wellbeing services

We’re looking for someone who:
- Has experience in healthcare, social care or a patient‑facing support role
- Is compassionate, organised and confident communicating with people
- Enjoys working as part of a team across different services
- Has good IT and admin skills and can manage a varied workload
- Understands the importance of confidentiality and safeguarding
You don’t need prior PCN experience — we’re interested in transferable skills and the right values.
