Summary
We are seeking a dedicated Support Worker to join our stable and experienced team at Hollybank House. This role is central to ensuring that each resident receives personalised, consistent support that enhances their wellbeing, independence, and quality of life. The position includes 32 contracted hours per week and 2 sleep‑ins, supporting the smooth running of our settled service.
Responsibilities
- Support the development, review, and implementation of personalised care and support plans.
- Provide daily assistance with personal care, communication, community access, and meaningful activities.
- Promote independence and positive outcomes through consistent, person‑centred practice.
- Supervise and guide support staff, ensuring high standards of care and professional conduct.
- Maintain clear, accurate records of support provided, progress, and any changes in need.
- Communicate effectively with residents, families, external professionals, and colleagues.
- Contribute to a positive team culture and participate in ongoing training and development.
- Uphold safeguarding, health and safety, and regulatory requirements at all times.
Requirements
- Previous experience in a senior support role or similar position within LD services is preferred.
- Strong understanding of person‑centred planning, positive behaviour support, and LD care practices.
- Confident communication skills and the ability to build positive relationships.
- Ability to supervise, motivate, and support a small team.
- Basic IT skills for record‑keeping and communication.
- A caring, reliable, and professional approach to supporting adults with learning disabilities.
- On call on a shared rota basis
