Job Title: Outreach Care and Support Worker

About the role
We are looking for compassionate, reliable and motivated Outreach Care and Support Workers to provide high quality, person-centred support to adults living in their own homes and accessing their local communities.
The role focuses on helping adults maintain their independence, make informed choices, build confidence and achieve agreed personal outcomes. You will work alongside individuals, their families and other professionals while maintaining dignity, privacy, confidentiality and appropriate professional boundaries.
Key responsibilities
- Provide individualised support in line with each person’s support plan, assessed needs, risk assessments, preferences and agreed outcomes.
- Promote independence by encouraging individuals to complete tasks themselves wherever possible, make their own choices and remain involved in decisions about their support.
- Support individuals with community activities, including appointments, shopping, education, employment, volunteering, social activities and accessing local services.
- Provide emotional support and reassurance while respecting each person’s culture, religion, identity, lifestyle, relationships and communication needs.
- Where required, assist with personal care, dressing, meal preparation, nutrition, hydration, household tasks, mobility and the safe use of equipment.
- Provide medication support within your level of training, competency and authorisation, maintaining accurate records and reporting any errors, refusals, omissions or concerns.
- Recognise and report safeguarding concerns, changes in health or wellbeing, environmental risks, accidents and incidents through the correct procedures.
- Seek consent before providing support and work in accordance with the Mental Capacity Act 2005 and any legally documented best-interest decisions.
- Complete accurate, factual and timely electronic care records, including support provided, choices made, outcomes achieved and actions taken.
- Follow lone working, health and safety, confidentiality, data protection and professional boundary procedures.
- Work collaboratively with individuals, families, colleagues and relevant health and social care professionals.
- Attend scheduled visits punctually and participate in required training, supervision, meetings and competency assessments.

Desirable experience and qualifications
- Previous experience in outreach, adult social care, supported living, domiciliary care or a similar setting.
- Care Certificate or a Level 2 or Level 3 qualification in Health and Social Care.
- Experience supporting adults with dementia, learning disabilities, physical disabilities, mental health needs or complex support requirements.
- Knowledge of adult safeguarding and the Mental Capacity Act 2005.
- Experience of lone working and using electronic care recording systems.
- Full UK driving licence and access to suitable transport.

Additional requirements
This is a community based position involving travel between individuals’ homes and community locations. Applicants must be willing to work flexibly, including evenings, weekends and bank holidays where required.
