About the Role:
We are looking for a Social Care Support Worker to provide practical, emotional and day-to-day support to adults who may need assistance to live safely and independently. You will build trusted relationships with service users, help them develop everyday skills and work with families and other professionals to deliver personalised support.
Key Responsibilities:
- Provide person-centred support in clients' homes and within the local community.
- Assist individuals with daily routines, appointments and community activities.
- Support people to maintain independence and develop practical life skills.
- Monitor wellbeing and report changes or concerns to senior staff.
- Work with families, carers and other professionals to coordinate appropriate support.
- Maintain accurate and confidential care records.
- Follow individual support plans and contribute to their regular reviews.
- Encourage participation in social, educational and community activities.
- Identify safeguarding concerns and follow appropriate reporting procedures.
- Promote dignity, choice, independence and respectful communication.
- Support individuals with learning disabilities, physical disabilities, mental health needs or age-related support requirements.
What We Are Looking For:
- Previous experience in social care, community support, family support or a similar role.
- Good understanding of person-centred care and safeguarding.
- Strong communication and interpersonal skills.
- Patient, reliable and able to build positive relationships.
- Ability to work independently and as part of a wider support team.
- Good record-keeping and organisational skills.
- Relevant qualification in Health and Social Care is advantageous.
- Care Certificate, NVQ or equivalent experience would be beneficial.
Pay: £2,500.00-£2,780.00 per month
