Requirements
- Highly motivated, ambitious and passionate about delivering high-quality care services
- Strong attention to detail and accuracy when managing schedules and reports
- Ability to work collaboratively with Care Managers, care workers and other team members
- Previous experience as a Care Coordinator within Health & Social Care (minimum of 2 years)
- Experience with electronic care planning and rostering systems
- Proven ability to manage and maintain a high-quality, dynamic care service
- Excellent people management and communication skills
- Strong leadership and teamwork abilities
- NVQ Level 3 in Health & Social Care, or willingness to work towards it
Key Responsibilities
- Ensure care worker visits are delivered in line with MyLife’s commitment to continuity and flexibility.
- Ensure all client visits take place on time and are allocated to the most appropriate care workers based on individual needs.
- Provide care workers with all necessary and up-to-date information regarding client visits, service delivery expectations and company policies.
- Receive and record information and reports from care workers, both verbally and in writing, within agreed timelines.
- Coordinate and schedule care worker review meetings and team meetings, ensuring they take place as planned.
- Cross-check actual visits against planned bookings to ensure accuracy and consistency in the rota.
- Support ongoing improvements in service monitoring, quality assurance and client safety.
- Assist Care Managers in communicating daily updates, visit changes and key information to care workers.
- Manage incoming phone calls and text messages, taking messages and redirecting communications as appropriate through the telephone system.
