Overview
We are seeking a compassionate and dedicated Community Care and Support Worker to join our team in Chester and Northwich. This role involves providing personalised care and assistance to individuals within their own homes or care settings, promoting independence and well-being. The ideal candidate will have excellent communication skills, a caring attitude, and the ability to work effectively both independently and as part of a team. Prior experience in care homes or home care is advantageous, along with a valid driving licence.
Responsibilities
- Deliver personalised care in accordance with individual care plans, ensuring the needs and preferences of clients are met with dignity and respect
- Assist clients with daily activities such as personal hygiene, dressing, meal preparation, and mobility support
- Support clients in maintaining their independence by encouraging participation in daily routines and social activities
- Maintain accurate records of care provided, observations, and any changes in client conditions
- Communicate effectively with clients, families, and healthcare professionals to ensure coordinated care
- Drive between different locations as required, ensuring safe transportation for clients when necessary
- Uphold health and safety standards at all times, adhering to organisational policies and procedures
Qualifications
- Valid driving licence with reliable access to a vehicle is preferred
- Experience working in a care home or providing home care services is desirable
- Knowledge of developing and implementing care plans tailored to individual needs
- Proficiency in English, both written and spoken, for effective communication
- Basic IT skills for maintaining accurate documentation and records
- Strong communication skills with the ability to build rapport with clients and colleagues
- Compassionate nature with a genuine desire to support vulnerable individuals
