Responsibilities
- Support patients or residents with personal care tasks such as bathing, dressing, grooming, and mobility assistance
- Assist with the preparation and administration of care plans tailored to individual needs
- Maintain clear communication with team members, patients, and their families to ensure consistent care delivery
- Help with medication reminders and monitor health status under supervision
- Drive or accompany patients for appointments or outings when required, ensuring safe transportation
- Document care activities accurately using IT systems and uphold confidentiality at all times
- Contribute to creating a warm, respectful environment that promotes dignity and independence
Skills
- Valid driving license with the ability to drive safely and responsibly
- Experience working in a care home or providing home care services is highly desirable
- Strong communication skills in English to effectively interact with patients, families, and team members
- Knowledge of care plans and ability to follow detailed instructions precisely
- Basic IT skills for documentation, record keeping, and accessing electronic health records
- Excellent interpersonal skills with a caring attitude and active listening abilities
- Ability to work collaboratively within a multidisciplinary healthcare team
