We are currently seeking an experienced Care Coordinator to join our growing team. The successful candidate will play a key role in ensuring the smooth day-to-day coordination of care services, supporting both clients and care staff to maintain exceptional standards of service delivery.
Key Responsibilities
- Coordinate daily care visits and rotas, ensuring efficient allocation of carers to meet client needs.
- Maintain regular communication with clients, families, and care teams to ensure satisfaction and continuity of care.
- Manage care schedules, ensuring all calls are covered and changes are communicated promptly.
- Participate in on-call duties on a rota basis to provide out-of-hours support.
- Maintain accurate and up-to-date care records in line with CQC and company standards.
- Ensure compliance with company policies, health and safety, and safeguarding procedures.
- Work closely with management to identify areas for service improvement and growth.
Requirements
- Previous experience as a Care Coordinator within domiciliary or community care is essential.
- Strong organisational and time-management skills.
- Excellent communication and interpersonal abilities.
- Good IT literacy and experience using care management software.
- A proactive and solution-focused approach.
- Understanding of CQC regulations and compliance requirements.
