What We’re Looking For:
Essential
- A full UK driving license and access to your own vehicle.
- The right to work in the UK.
- Previous experience as a care coordinator, team leader or supervisor
- Strong knowledge of person-centred care and the needs of vulnerable adults
- Skilled in operating care management and rota management systems
- Able to work under pressure and manage competing priorities
- Reliable, adaptable and resilient
- Excellent communication and organisational skills
- Committed to upholding the values of dignity, respect and inclusivity
Desirable
- NVQ/QCF Level 3 in Health and Social Care or equivalent
- Proficient with CQC regulations and compliance requirements

Your Role:
As a Care Coordinator, you will be reporting to the Registered Manager. You are to ensure the smooth and efficient delivery of high-quality domiciliary care services by coordinating care packages, managing staff rotas and maintaining strong relationships with clients, families and care workers. This role ensures clients receive person-centred care while adhering to all relevant regulations, policies and standards.
Key Responsibilities:
- Coordinate and schedule care visits to ensure effective service delivery.
- Prepare and manage staff rotas, ensuring all care visits are covered.
- Respond to changes in care needs and staffing requirements.
- Liaise with clients, families, care staff, and healthcare professionals.
- Assist with care plans, risk assessments, and new client referrals.
- Monitor staff compliance, attendance, and mandatory training.
- Maintain accurate care records and management systems.
- Support recruitment, induction, and supervision of care staff.
- Handle enquiries, incidents, complaints, and safeguarding concerns.
- Ensure compliance with CQC regulations, company policies, and participate in the on-call rota.
