Spring Hall Practice Group is a well-established and growing GP Practice Group with five surgeries, and we are seeking a dedicated and compassionate Practice Nurse to join our friendly and supportive primary care team.

About the Role
The successful candidate will be a highly motivated and proactive Practice Nurse who brings a positive, "can-do" attitude and thrives within a collaborative team environment. You will be clinically competent, adaptable, and committed to delivering the highest standards of patient-centred care.
Working as part of our multidisciplinary team, you will provide high-quality nursing care to a diverse patient population while confidently managing your own workload and supporting colleagues when required. You will contribute to service development, quality improvement initiatives, and help maintain the excellent standards our patients expect.
You will work closely with our GPs, Advanced Clinical Practitioners, Practice Nurses, Nursing Associates, Healthcare Assistants, Pharmacists, and Administration teams to provide seamless, coordinated care. You will also support colleagues through mentoring, sharing knowledge, and contributing to the ongoing development of the nursing team.
Key Responsibilities
- Deliver evidence-based nursing care within primary care.
- Undertake annual reviews and long-term condition management for patients with:
- Diabetes
- Asthma
- COPD
- Hypertension
- Cardiovascular disease
- Carry out blood pressure monitoring and cardiovascular risk assessments.
- Provide lifestyle advice, including:
- Smoking cessation
- Weight management
- Healthy living advice
- Support patients to self-manage long-term conditions through education, care planning, and personalised action plans.
- Deliver cervical screening as part of the National Cervical Screening Programme, including:
- Performing cervical smear tests
- Providing patient education and reassurance
- Maintaining accurate records and arranging appropriate follow-up for abnormal results
- Administer childhood and adult immunisations in accordance with national guidance.
- Provide wound care and dressings.
- Deliver contraception and sexual health services within scope of practice.
- Undertake health promotion, NHS Health Checks, and screening programmes.
- Support GPs with minor surgical procedures where required.
- Maintain accurate clinical records using the practice clinical system.
- Work in accordance with CQC standards, infection prevention policies, and professional nursing regulations.
About You
The successful candidate will have:
- Registered Nurse (NMC) qualification with an active NMC registration.
- Experience working within Primary Care or General Practice is desirable.
- Experience managing long-term conditions.
- Competence in cervical cytology and immunisations (or willingness to undertake training).
- Excellent clinical assessment and communication skills.
- A proactive and flexible approach to patient care.
- The ability to work independently while contributing positively within a multidisciplinary team.
- A commitment to continuous professional development and quality improvement.
- Excellent organisational skills and attention to detail.
