Senior Housing Support Worker
We are looking for an experienced Senior Housing Support Worker to join our team at Tate Housing.
Tate Housing provides accommodation and support to people facing homelessness. We currently operate multiple properties across Nottingham City and are continuing to expand.
The successful candidate will provide direct support to clients while offering day-to-day guidance and oversight to the support team.
Duties and responsibilities:
- Managing a caseload and supporting clients with a range of needs and challenges.
- Processing and reviewing incoming referrals.
- Completing referral assessments and gathering information from relevant agencies.
- Assessing applicants’ needs, risks and suitability for the service.
- Liaising with referral agencies throughout the assessment and placement process.
- Supporting new clients through the sign-up and move-in process.
- Completing and regularly reviewing support plans and risk assessments.
- Conducting weekly support sessions focused on clients’ identified needs and goals.
- Providing day-to-day guidance and support to Housing Support Workers.
- Monitoring staff caseloads, appointments and the quality of support records.
- Reviewing client files to ensure documentation is accurate, complete and up to date.
- Ensuring room inspections, welfare checks and fire safety checks are completed as required.
- Supporting staff with complex cases, incidents and safeguarding concerns.
- Escalating serious concerns, risks and incidents to management promptly.
- Attending appointments with clients when required.
- Signposting and referring clients to appropriate external services.
- Liaising with probation, local authorities, healthcare professionals and other agencies involved in clients’ support.
- Attending and contributing to multi-agency meetings.
- Supporting the induction and development of new and existing staff.
- Promoting professional boundaries and ensuring policies and procedures are followed.
- Supporting the manager with service monitoring, audits and continuous improvement.
- Managing time effectively and organising your own caseload, appointments and calendar.
- Travelling regularly between Tate Housing properties.
The successful candidate must have strong communication, organisational and leadership skills, with previous experience in supported housing, homelessness, social care or a similar environment.
A full driving licence and access to a car are essential for this role.
Job Types: Full-time, Part-time
Pay: £28,000.00 per year
