Job Description: Support Worker
Location: Bracknell Area
Responsible to: HR Manager
Overview:
We are currently looking for support workers to work in the Bracknell area on either a part-time or full-time basis.
As a Support Worker you will be a key member of our team, providing high standards of person-centered support and care to our service users. Through a creative and caring approach, you will build good relationships with both service users and their families, supporting service users in the planning of goals and in living as independently as possible whilst ensuring their safety. Actively engaging and participating in activities is often key to the role, so energy and initiative are vital skills; experience however is not essential, providing you have a willingness to learn and the compassion and patience needed to support.

We deliver approximately 1,500 hours a week of innovative care-in-the-community to over 130 vulnerable adults in the Wokingham, Bracknell and Reading area.
Essential Requirements:
· A positive attitude towards people using support
· The ability to work independently
· Good verbal and written communication skills
· A willingness to train in essential areas
· A methodical and mature approach to solving problems, scoping options and looking at alternatives
· Patient, thorough, determined, organised, friendly and approachable, strong team player, committed to the care industry
· Appointments will be subject to a satisfactory DBS (criminal record) check.
· Driver with access to own vehicle and happy to take clients in your car (will require business car insurance)
Responsibilities:
· Undertake all direct support and care tasks as required by Support Horizons and their customers
· Complete records and reports as required by Support Horizons and the person supported
· Contribute to the fulfilment and development of Support Horizons customers
· Follow the support or care plans and any related guidelines of the people being supported
· Undertake training in line with Support Horizons and CQC requirements
· Attend staff team meetings and participate in supervisions and appraisals as required
· Be aware of and follow all Support Horizons policies and procedures and guidelines
· Maintain the confidentiality of information relating to Support Horizons and the people we support
· Comply with the Care Quality Commission Standards and any other relevant legislation relating to a customer
· Liaise with customers, families and relevant professionals as directed and required by Support Horizons
· Inform Support Horizons of any problems, concerns or issues relating to a customer, their support or safety
· Maintain personal awareness of Support Horizons business and undertake training as necessary
· Uphold and promote the ethics and social values of Support Horizons
· In accordance with the provisions of the Health and Safety at Work Act 1974 take reasonable care for the health and safety of yourself and of other persons who may be affected by your acts or omissions at work; and cooperate with Support Horizons to enable the organisation to perform of comply with its duties under statutory health and safety provisions.

Pay: From £12.71 per hour
