The Mental Health Hospital Discharge Team is recruiting for 2 Full Time Social workers.
You will have a Social Work qualification, and registration with Social Work England. The right should have experience of working with mental disorder, pre or post qualifying.
You should have experience of working in a multi-disciplinary way, be able to work quickly, proportionately and have confidence in their decision making, and positive risk taking.
You should have a reasonable working knowledge of the Mental Health Act, the Mental Capacity Act and Care Act 2014.
Mental Health Hospital Discharge
We are a small team of social workers, located at City Hall, but with an office space available at Callington road Hospital, and the opportunity to hot desk throughout other city office bases. The main function of the team is to facilitate timely discharges from mental health hospitals. We work closely with the psychiatric wards to coordinate safe discharges from hospital.
The work is fast-paced and varied, with a strong focus on person centred, multi-disciplinary working.
Many of our service users experience multiple disadvantages, including homelessness, substance abuse and forensic histories. Therefore, you must be prepared to work with several complex cases simultaneously.
We work with the acute wards at Callington Road Hospital, Weston General Hospital and Southmead Mental Health Wards.
Please note this role is subject to an Enhanced DBS Check.
