Job Purpose
To provide a high standard of nursing and palliative care, meeting the individual needs of residents based on comprehensive assessments undertaken in consultation with residents, their families, and the multidisciplinary team. The Registered Nurse is responsible for assessing, planning, implementing, and evaluating person-centred care while promoting residents’ dignity, independence, comfort, and wellbeing.
Key Responsibilities
- Provide high-quality, person-centred nursing and palliative care to residents in accordance with individual care plans and professional standards.
- Assess residents’ physical, psychological, emotional, and social needs, developing, implementing, and regularly reviewing individualised care plans.
- Reassess residents’ changing needs and adapt care plans, accordingly, ensuring care is prioritised appropriately.
- Administer medications safely and in accordance with current legislation, NMC standards, organisational policies, and best practice.
- Order medications and medical supplies and ensure the safe storage, administration, recording, and disposal of medicines, including controlled drugs, in line with accepted procedures.
- Monitor residents’ physical and mental health, recognising changes in condition and taking appropriate clinical action.
- Carry out routine clinical observations and maintain accurate, confidential, and up-to-date electronic care records.
- Deliver evidence-based nursing care, including: Wound assessment and management, Percutaneous Endoscopic Gastrostomy (PEG) care and administration of enteral feeds, Stoma care, Catheter care, Diabetes management, End-of-life and palliative care.
- Respond promptly and effectively to medical emergencies.
- Delegate duties appropriately to Care Assistants and other members of the care team, providing supervision, support, and guidance to ensure safe and effective care delivery.
- Communicate effectively with residents, relatives, General Practitioners (GPs), hospitals, pharmacists, and other healthcare professionals to ensure continuity of care.
- Participate in multidisciplinary meetings and contribute to care planning and clinical decision-making.
- Promote infection prevention and control measures throughout the home.
- Identify and report safeguarding concerns in accordance with organisational policies and statutory requirements.
- Participate in clinical audits, quality improvement initiatives, and risk assessments.
- Support residents to maintain their dignity, independence, choice, and quality of life.
- Mentor newly appointed staff and student nurses where appropriate.
- Participate in effective shift handovers to ensure continuity of care.
Professional Responsibilities
- Maintain current registration with the Nursing and Midwifery Council (NMC) and practise in accordance with the NMC Code.
- Maintain professional competence through Continuous Professional Development (CPD).
- Adhere to all organisational policies and procedures, including those of The Charlotte Straker Project.

Person Specification
Essential
- ·Registered Nurse with a valid NMC registration (RGN, RMN, or RNLD).
- Demonstrable clinical competence in nursing practice.
- Excellent assessment, care planning, and clinical decision-making skills.
- Ability to safely administer medicines and maintain accurate clinical records.
- Strong communication and interpersonal skills.
- Ability to supervise, delegate, and support care staff effectively.
- Compassionate, caring, and committed to delivering person-centred care.
- Good organisational and time management skills.
- Ability to work independently and as part of a multidisciplinary team.
Desirable
- Experience working within a care home, hospice, or community setting.
- Experience in palliative and end-of-life care.
- Experience caring for residents with dementia and other complex health conditions.
- Leadership or supervisory experience.
- Knowledge of clinical governance, safeguarding, infection prevention and control, and quality assurance.
