Job summary
We are looking for an enthusiastic and passionate Registered Nurse to join the Hampshire Tissue Viability Team.
Our mission is to provide an open and honest, holistic, collaborative, specialist tissue viability service that facilitates the highest quality, evidence-based care, which keeps our patients at the heart of everything we do.
You will be based at Cromwell House in Winchester, covering the mid Hampshire area including Chandlers Ford and Eastleigh. There may be occasions where you are asked to travel further within Hampshire to support service delivery or attend meetings.
Our role is to support health care professionals in clinical practice by providing teaching, information and specialist advice on tissue viability.
We provide:
Complex wound assessment and advice
Leg ulcer service, including lower limb assessment
Telephone advice
Training and education
Supervision and competency review
Link nurse meeting and training
The successful candidate will support patients and their families to achieve improved health outcomes and promote health and wellbeing, while demonstrating care, compassion, competence, communication, courage, and commitment.
Care is based on comprehensive assessments and is patient/client centred, culturally sensitive, evidence-based and promotes independence.

Main duties, tasks & skills required
- To work within the Tissue Viability Specialist Team across a designated geographical area and support the delivery of specialist care, advice and education to a range of staff.
- To provide high levels of knowledge and skill in tissue viability care.
- To provide support to Integrated Community Care Teams, GP practices and other members of the Multi-Disciplinary Team in ensuring the delivery of evidence based, high quality tissue viability care to patients.
- To work in clinic settings, patients' homes, GP surgeries and in residential care settings to support general clinical staff in providing tissue viability care
- To participate in the investigation process related to pressure ulcer incidence and provide expert input in the SI process.
- To act as a role model and expert in the prevention, treatment and care of tissue viability conditions
- Responsible for providing structured, formal and informal education through a variety of media including clinical coaching, formal teaching sessions and role modelling in all aspects of evidence-based tissue viability care.
- To support Pressure Ulcer Panels and Tissue Viability Advice Line as part of rota and cover other colleagues' area as required.
