An exciting opportunity has arisen for a Clinical Pharmacist to join Erewash Primary Care Network (PCN), supporting our network of forward-thinking General Practices covering circa 100,000 patients in the Erewash Valley and surrounding areas. The successful candidate will work collaboratively with GP practices to deliver a high-quality clinical pharmacy service, improving medicines optimisation and patient outcomes across the PCN.
Working as part of a multidisciplinary team, you will collaborate closely with colleagues including Community GPs, Advanced Clinical Practitioners (ACPs), and members of the Ageing Well team to provide proactive, patient-centred care.

Main duties of the job
The post holder is a Clinical Pharmacist who will work within their professional boundaries to provide a high-quality clinical pharmacy service across Erewash Primary Care Network. The successful candidate will work collaboratively with GP practices within the network to deliver patient-focused medicines optimisation, improve patient outcomes, and support the safe and effective use of medicines.
The post holder will work alongside other Clinical Pharmacists and the wider multidisciplinary team, including GPs, Advanced Clinical Practitioners, and other healthcare professionals. They will be supported by Community GPs within member practices and a Senior Clinical Pharmacist who will provide mentorship, clinical support, and opportunities for professional development.
The role will involve undertaking structured clinical medication reviews, supporting long-term condition management, addressing medicines-related queries, and proactively managing patients, including those with complex polypharmacy and multiple long-term conditions.
The post holder will provide expert support to practices with prescription and medication queries, supporting safer prescribing systems, medicines reconciliation following transfer of care, repeat prescribing processes, and acute prescription requests. They will provide clinical medicines advice to patients and healthcare professionals, helping to improve access to pharmacy expertise within primary care.
The post holder will contribute to medicines optimisation, quality improvement initiatives, and delivery of PCN priorities, including supporting aspects of the Quality and Outcomes Framework (QOF) and enhanced services.
The role will also support closer integration between general practice, community pharmacy, and secondary care services, helping to maximise skill mix, improve patient outcomes, and enhance the efficiency and quality of medicines-related processes.
Experience of working with older people, frailty services, or within an Ageing Well model would be desirable, with the opportunity for the successful candidate to develop a specific frailty-focused role within the PCN. This may include supporting proactive care for older adults with frailty, complex needs, and medicines optimisation challenges.
The role is pivotal in supporting the continued development of the PCN clinical pharmacist service and requires an enthusiastic, motivated pharmacist with a passion for delivering excellent patient care within a forward-thinking primary care environment.
Overview of your organisation
Erewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of six constituent practices (see below). EHP employs approximately 250 people who work within the six constituent practices and within the EHP central team, which provides support services e.g., finance, operational, data & performance etc.
As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.
EHP is governed by the EHP Senior Leadership Team reporting to the EHP Board which meets quarterly. The Board currently consists of one GP Partner from each constituent practice and the EHP Managing Partner & CEO, with other colleagues in attendance.

EHP consists of 6 GP constituent practices who are contracted to deliver primary care services to their registered patient population through a GMS or PMS contract. The practices range in size from approximately 3,500 patients to 19,000 patients.

Qualifications Essential
Mandatory registration with General Pharmaceutical Council
Masters degree in pharmacy (MPharm) (or equivalent)
Minimum of 2 years post-qualification experience.
An appreciation of the nature of GPs and general practices.
An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Excellent interpersonal, influencing and negotiating skills
Excellent written and verbal communication skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
