Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team in providing exceptional care within our dual services including domiciliary care and supported living for adults. The ideal candidate will possess a strong background in care, including experience in home care and supported living environments. This role is pivotal in supporting the management of daily operations, ensuring the highest standards of care for our service users. You will be expected to travel to service user’s houses to take part in review meetings, so a full driving license is essential and your own vehicle.
Responsibilities
- Oversee rotas for our domiciliary care service and supported living
- Develop and review care plans and risk assessments
- Attend review meetings
- Assess new service users and induct them into the service
- Attend staff interviews and play a role in recruitment
- Continually develop within role into a leadership role where appropriate
- Take part in on call rota
- Undertake competency assessments for staff members.
- Be adaptable to change and working in a fast-paced environment.
- To take part in completing reports and promoting good governance, as delegated.
It is expected that this role will involve direct care and support part time and administration part time as a dual role.
Experience
- Proven experience in care settings, preferably in roles such as administration within a home care or supported living environment.
- Excellent communication skills, both verbal and written, with an emphasis on compassionate interaction with service users and their families.
- Ability to develop comprehensive care plans that address the unique needs of each service user.
- Strong scheduling and organisation skills.
- Understanding of person centred care and care planning and risk assessments.
- Calm solution focused decision making.
Must hold an NVQ Level 3 in Health & Social Care or must be willing to work towards achieving this qualification
