Wells Road Surgery is a family orientated GP surgery serving a diverse local patient population. We are small enough to provide compassionate, personal primary care – while also being supported by wider specialists offering more focussed treatment. The Care Navigator team is at the heart of the practice, co-ordinating access and care to directly support the health and wellbeing of our patients.
Our Care Navigators are responsible for guiding and communicating with our patients to achieve the best health outcomes possible.
A Care Navigator at Wells Road Surgery will:
- After training, triage online medical and administrative requests and guide patients on the most appropriate care.
- Manage clinical appointments, working with a team of skilled doctors, nurses, and wider specialists.
- Support the practice with a range of administrative duties.
Further responsibilities may be assigned as the practice team develops.
Job Types: Part-time, Permanent
Pay: £12.96 per hour
