An exciting opportunity has arisen for an experienced and innovative Allied Health Professional to join the Integrated Community Stroke Service (ICSS) and Stroke and Neurology Community Rehabilitation Team as the Digital Rehabilitation Lead .
This is a specialist leadership role responsible for driving the development, implementation and evaluation of digital rehabilitation across stroke and neurological services. The post holder will combine a highly specialist clinical caseload with service development responsibilities, ensuring patients have equitable access to high-quality, evidence-based rehabilitation through both traditional and digital models of care.
Working as part of a multidisciplinary team, the Digital Rehabilitation Lead will provide advanced assessment and treatment for patients with complex stroke and neurological rehabilitation needs, while leading the adoption of innovative digital solutions that enhance patient outcomes, improve access to services and support service transformation.
The successful candidate will be an experienced Physiotherapist, Occupational Therapist or Speech and Language Therapist with a strong background in stroke and neurological rehabilitation, excellent leadership skills and a passion for innovation and service improvement.
This role offers the opportunity to influence the future of community rehabilitation services across Cornwall, working collaboratively with colleagues, patients, carers and partner organisations.
The Digital Rehabilitation Lead will:
Lead the development, implementation and continuous improvement of digital rehabilitation services across stroke and neurological rehabilitation pathways.
Act as a clinical specialist, providing advanced assessment, diagnosis, treatment planning and rehabilitation for patients with complex stroke and neurological conditions.
Deliver expert clinical interventions and provide specialist advice to patients, carers, multidisciplinary colleagues and partner agencies.
Support the delivery of innovative rehabilitation programmes using digital technologies, tele-rehabilitation platforms and virtual models of care.
Provide professional and clinical leadership to colleagues, supporting workforce development, supervision, teaching and mentorship of registered and non-registered staff.
Promote evidence-based practice by supporting audit, quality improvement, service evaluation and research activities.
Work collaboratively with multidisciplinary teams, acute services, primary care, social care, voluntary sector organisations and other stakeholders to ensure seamless patient pathways.
Identify opportunities to improve service delivery, patient experience and rehabilitation outcomes through digital innovation and transformation.
