The diabetes team is based in the acute hospital trust and consists of a Consultant Diabetes nurse, 2 advance nurse practitioner's, and Diabetes specialist nurses working in outpatient clinics, in-patients and community . We work closely as a multi-disciplinary team including our administration team and the consultant diabetologists providing a Monday to Friday service. We have a well established insulin pump service over both hospital sites .
Provide leadership in the field of diabetes and draw on advanced knowledge and exercise professional skills to develop and deliver high quality service to people with diabetes.
- Act as an expert practitioner, overseeing, delegating and supporting care carried out by other members of the diabetes team where required
- Work closely with other departmental staff and professionals to facilitate a multidisciplinary framework for the effective delivery of diabetes care.
- Work collaboratively and co-operatively with clinical colleagues to develop an innovative high quality service, which enables individualised care to be facilitated and delivered to people with diabetes and their carers/families.
- Responsible for delivering an equitable nurse lead service for people with diabetes in Warrington and Halton.
- Be responsible for a defined clinical caseload of people living with Type 1 and Type 2 diabetes.
- Make an assessment on complex facts to formulate a care plan, implement and continual evaluate care.
- Review patient’s medication regime ensuring that patients receive appropriate pharmacotherapy in effective doses. Prescribe changes or initiate medication in accordance with non medical prescribing.
- Refer people to relevant practitioners or agencies when this is necessary providing accurate and necessary information consistent with maintaining confidentiality.
