We’re looking for a Support Coordinator to join our friendly and dedicated team based at Kings Mill Hospital to deliver a vital service that supports individuals following their discharge from hospital.
This is a rewarding opportunity to help people regain their independence, reduce the risk of readmission, and improve their overall well-being. You’ll be at the heart of a service that ensures no one feels alone or unsupported after leaving hospital.
What You’ll Be Doing
- Visiting patients in their homes shortly after discharge to check on their well-being and ensure they have essentials like food, heating, clean bedding, and medication.
- Carrying out initial and follow-up assessments using the Outcomes Star tool to monitor progress and identify support needs.
- Coordinating with hospital discharge teams, reablement services, families, and volunteers to deliver a seamless support plan.
- Deploying volunteers and arranging practical help such as transport, shopping, or companionship.
- Maintaining accurate records and contributing to service reports and evaluations.
What We’re Looking For
- A background in health, social care, or community support (qualifications or equivalent experience).
- Strong interpersonal and communication skills, with the ability to build trust and rapport quickly.
- A flexible, empathetic, and person-centred approach to support.
- Confidence in working independently and as part of a wider team.
- Good IT and record-keeping skills.
- A full UK driving licence and use of your own vehicle (mileage reimbursed).
- Willingness to undergo an Enhanced DBS check (arranged and paid for by us).
