Symbiotic Homecare is a medically led domiciliary and complex-care provider delivering high-quality care across Lancashire.
We support individuals with a wide range of needs, from traditional domiciliary care through to complex community packages involving neurological conditions, acquired brain injury, spinal cord injury, dementia, learning disability, epilepsy, enteral feeding, respiratory support, palliative care and end-of-life care.
Our service is supported by Consultant Medical Directors, Registered Nurses and specialist clinicians, providing a strong clinical governance and escalation structure.
As our complex-care services continue to grow, we are looking for an experienced Registered Nurse to join our senior management team as Clinical Operations Manager.
The Role
This is a hybrid clinical and operational leadership position.
The successful candidate will support the safe and effective day-to-day running of the service while also providing registered nursing oversight across our complex-care packages.
You will work closely with the Registered Manager, Directors, Medical Directors, clinical team, field managers, care coordinators and external healthcare professionals.
The role is mainly office-based in Blackburn, with regular visits to clients across Lancashire to assess staff competency. Mileage reimbursed.
Key Responsibilities
You will:
- Support the day-to-day management of domiciliary and complex-care services
- Provide leadership and support to coordinators, field managers, supervisors and care staff
- Participate in new-client assessments and mobilisation of complex-care packages
- Develop and review care plans, risk assessments and clinical documentation
- Ensure staff are appropriately trained and competent before undertaking complex healthcare activities
- Deliver and support clinical training for care staff
- Undertake staff observations, practical competency assessments and sign-offs where appropriately trained and competent
- Maintain oversight of clinical competencies and reassessment requirements
- Support medication governance, safeguarding and clinical incident management
- Review incidents, complaints and concerns and help implement lessons learned
- Undertake staff supervision, appraisal, spot checks and clinical reviews
- Support recruitment, induction and ongoing staff development
- Liaise with GPs, District Nurses, specialist nurses, hospitals, CHC teams and other MDT professionals
- Support CQC compliance, clinical governance, audits and quality improvement
- Identify deterioration or clinical risk and ensure appropriate escalation
Complex Care & Clinical Competency
Depending on your experience and competence, the role may include oversight, training and competency assessment in areas such as:
- PEG and enteral feeding
- NG tube care
- Medication administration
- Epilepsy and rescue medication
- Oxygen therapy
- NIV and home ventilation
- Tracheostomy care
- Tracheal and oral suction
- Cough Assist
- Catheter and suprapubic catheter care
- Stoma care
- Complex moving and handling
- Pressure-area management
- Neurological conditions
- Spinal cord injury
- Acquired brain injury
- Motor neurone disease
- Learning disability and autism
- Palliative and end-of-life care
We do not expect applicants to already be competent in every area. Further specialist training and professional development can be provided.
Field-Based Clinical Oversight
Regular field visits will include:
- New-client and complex-care assessments
- Staff observations and spot checks
- Clinical competency assessments
- Medication competency assessments
- Care-plan reviews
- Clinical supervision
- Post-incident reassessment
- Package mobilisation
- Meetings with clients and families
- Joint visits with specialist clinicians where required
Travel costs will be reimbursed.
Essential Requirements
Applicants must:
- Be a Registered Nurse with active UK NMC registration
- Have post-registration nursing experience
- Have experience supporting individuals with complex healthcare needs
- Have strong clinical assessment and decision-making skills
- Be confident training, coaching and assessing care staff
- Understand safe delegation, professional accountability and scope of competence
- Have strong leadership and communication skills
- Be confident challenging unsafe or poor practice
- Have a good understanding of safeguarding and clinical risk
- Be organised and able to manage competing priorities
- Hold a full UK driving licence and have access to a vehicle
Previous experience in domiciliary care, community nursing, complex homecare, CHC, respiratory care, neurology, spinal injury, ABI, MND, palliative care or clinical education would be particularly advantageous.
Previous domiciliary management experience is desirable but not essential where the candidate demonstrates strong leadership ability.

Medical & Clinical Support
The successful candidate will work within a medically led governance structure and will have access to support from our Consultant Medical Directors, Registered Manager, Registered Nurses and specialist clinicians.
You will not be expected to manage difficult or unusual clinical issues in isolation.

Working Pattern
This is a full-time, permanent position based primarily from our Blackburn office, with regular travel across Lancashire.
Flexibility outside standard office hours will be required due to the nature of domiciliary and complex care.
The successful candidate will also form part of the senior escalation structure for the service.
